Decision Making Counseling Worksheets for Grades K-8 | Help Kids Make Decisions
Decision making worksheets, picture-based choice maps, and counselor guides that help students in grades K-8 slow down and make thoughtful choices. Ready for individual, small group, or classroom counseling.
Help students stop, think, and choose with these decision making worksheets, visual choice maps, and counseling guides for grades K-8. This ready-to-use set walks students through real, everyday decisions step by step. They name the problem, come up with more than two options, weigh realistic outcomes, think about how each choice will feel now and later, and take a small first step.
It’s designed for Tier 2 small groups and Tier 3 individual counseling, and every handout also works as a Tier 1 classroom or advisory tool with a shared practice scenario. It’s a strong fit for students who make impulsive choices, freeze when every option feels too big, see only “do it or don’t,” catastrophize outcomes, or look to adults to decide for them.
The set includes three developmentally matched student tools:
- My Choice Map (Grades K-2): a picture-based, draw-and-circle map for emerging readers, with bubble letter choices, line-drawn feeling faces, and a “sunny side, cloudy side” way to weigh options
- My Decision Guide (Grades 3-8): a one-page, nine-step guide that walks students through understanding, exploring, and deciding
- My Decision Map (Grades 5-8): a three-page trail that follows six stops plus a follow-up reflection, with space to compare options in depth
Every resource follows the same three-part decision making sequence:
- Understand: name the problem, notice the tug-of-war, and decide how big the decision really is
- Explore: list options, predict best, worst, and most likely outcomes, and consider feelings right away and later on
- Decide: check each option against values, safety, and kindness, then choose and plan a small first step

Frequently Asked Questions:
Is this for individual or group counseling?
Both. It’s designed for Tier 2 small groups and Tier 3 individual counseling. Each handout can also be modeled as a Tier 1 classroom or advisory lesson using a shared practice scenario, so no student has to share something personal.
Can I use it with students who can’t read yet?
Yes. My Choice Map was built for emerging-readers. Students draw and circle instead of writing, and the counselor guide includes read-aloud scripts for every step.
How do I know which option to use?
Choose by reading or developmental level. Emerging readers do well with My Choice Map, and stronger readers can use My Decision Guide with some read-aloud support.
Is it a good fit for impulsive students?
Yes. Sizing up the decision and comparing how a choice will feel right away with how it will feel later are built in to slow down hot, in-the-moment choices. The counselor guides include specific adaptations for impulsive decision-makers.
Does it include black-and-white versions?
Yes. Every page comes in both color and black-and-white.
How long does it take?
About 25 to 35 minutes for the Decision Map, or about 15 to 20 minutes one-on-one for the Choice Map. Both split easily across two sessions, with a short follow-up a week or two later.
